Outpatient Addiction Treatment
The New Found Life Outpatient Program offers ongoing treatment for clients in recovery from substance use disorder. The program is open-ended by design and provides clients who have completed residential treatment and are ready for a less intensive level of care with ongoing guidance and support. Outpatient services foster accountability and integrity as clients continue to learn, grow and implement recovery skills into their daily lives.

Continuing Case Management
The transition into outpatient treatment is a significant one. In the early stages of recovery, clients benefit from the ongoing support our outpatient services provide, including case management. Upon admission to New Found Life, each client is assigned a dedicated case manager who they will work with throughout the entire continuum of care, whether they join us through residential treatment, our Outpatient Program or Transitional Living.
A case manager serves as a valuable resource, a primary point of contact and an advocate for recovery as they help their client successfully navigate their recovery. Case managers get to know their clients and their needs over time. Recovery plans are subject to change at any given time, and it’s the case manager’s responsibility to make sure their client is participating in the best modalities to suit their needs.
The residential treatment setting is a safe place where you devote all of your time to healing and recovery, but the real work begins once you’re out of the traditional treatment setting. The transition back into the community can be full of challenges, which is why a case manager is such a valuable asset in recovery.
Client and case manager work together to:
- Develop an individualized outpatient treatment plan tailored the client’s unique circumstances.
- Set and achieve goals.
- Plan for life after treatment, which may include career or educational assistance.
- Design a relapse prevention plan.
- Locate resources, such as local 12-step groups, counselors or legal assistance.
